Ready to Get Started? Easy Steps

Easy Steps Ahead

Step 1:  Make sure you are in network by contacting your health insurance company and use your specific plan to get accurate information.

Step 2: Have your referring physician fax a recent referral to us with all pertinent notes regarding your condition including any tests, labs, scans…

Step 3: After we receive the referral AND notes regarding your condition with updated demographics, list of doctors and current and/or previous neurologists and medication list, we will contact you and send a Pre-registration online form to be completed in full.

Step 4: Once the Pre-registration is completed, we will contact you and make an appointment for you to see us.

Step 5: Waiting is hard…If there is a long wait for your appointment, we will place you on a cancelation list and make attempts to get you in sooner. (It does happen, because patients need to cancel and move their appointments for all sorts of reasons.)

Big things to know:

Big thing one: 20 minutes early every appointment every time…There is always something to sign and confirm at check-in even if you checked in online. We need to get you back to the room and ready to see the doctor and that takes time. Your appointment time is with Dr. Lifton and that 20 minutes gives us time to update information, take vitals, and welcome you to your visit and ask a few questions.  Please do not ask for an exception to this.

Big thing two: Insurance is tricky, and we do our best but we do focus on your healthcare and ask that you know your insurance so there are no surprises. Insurance will never guarantee coverage until they get a claim and review it.  So, please be aware of that nuance.

Why Are We Referral Based?

To ensure Dr.  Lifton can provide the precise, comprehensive care you deserve, we operate on a referral-based model.

Prior to starting the pre-registration process and scheduling your first appointment, we must first receive a formal referral and accompanying note from your referring physician.

This allows us to dedicate the necessary time to thoroughly review your full medical history and prepare for your specific neurological needs. Please ask your primary care provider to fax or send this documentation to our office.

Our FAX number is 941-485-2150.

Call our office at 941-485-2220 with any questions. If we are with other patients, please leave a detailed message on the voicemail system so we can get started after we receive your message.

Request an Appointment

If you request an appointment, it is good to follow up with a phone call to confirm we received it.
